What is happening?

Hysbysiad yn unol ag adran 24(3) o’r Ddeddf uchod.

Mae Ombwdsmon Gwasanaethau Cyhoeddus Cymru wedi ymchwilio i gwyn ac wedi canfod methiant gwasanaeth gan Fwrdd Iechyd Prifysgol Bae Abertawe. Canfu’r Ombwdsmon fethiannau yn ymwneud â’r ffordd y rheolwyd arhosiad y claf am lawdriniaeth. Yn benodol, y collwyd cyfle i’r claf gael llawdriniaeth orthopedig, o ganlyniad i ailosod cloc y rhestr aros yn amhriodol a diffyg cyfathrebu â’r claf mewn perthynas â hyn. 

Hoffai’r Bwrdd Iechyd ymddiheuro’n gyhoeddus am y methiannau a nodwyd ac am y gofid a achoswyd i’r claf. Gwnaed nifer o argymhellion i’r Bwrdd Iechyd gan yr Ombwdsmon, sydd wedi cael eu derbyn yn llawn a byddant yn cael eu gweithredu o fewn yr amserlenni penodedig.

Bydd copi o adroddiad yr Ombwdsmon ar ganlyniadau ei ymchwiliad ar gael ar wefan y Bwrdd Iechyd https://bipba.gig.cymru/ ac i’w archwilio gan y cyhoedd, yn ddi-dâl, yn ystod oriau gwaith arferol, ym Mhencadlys Bwrdd Iechyd Prifysgol Bae Abertawe, 1 Porthfa Talbot, Port Talbot, SA12 7BR am gyfnod o 3 wythnos o 27 Tachwedd 2025, i unrhyw un sy’n dymuno cael copi o’r adroddiad hwn neu gymryd dyfyniadau ohono. Darperir llungopïau o’r adroddiad neu rannau ohono yn am ddim ar gais.

PUBLIC SERVICES OMBUDSMAN (WALES) ACT 2019
Notice pursuant to s.24(3) of the above Act.

The Public Services Ombudsman for Wales has investigated a complaint and found a service failure by Swansea Bay University Health Board. The Ombudsman found failings relating to the way that the patient’s wait for surgery had been managed. In particular that there had been a lost opportunity for the patient to undergo orthopaedic surgery, as a result of an inappropriate waiting list clock re-set and lack of communication with the patient in relation to this.

The Health Board would like to publicly apologise for the failings identified and for the distress that this caused to the patient. A number of recommendations were made to the Health Board by the Ombudsman, which have been fully accepted and will be implemented within the prescribed timescales.

A copy of the Ombudsman’s report on the results of his investigation will be available on the Health Board’s website https://sbuhb.nhs.wales/ and for inspection by the public, without charge, during normal working office hours, at Swansea Bay University Health Board Headquarters, 1 Talbot Gateway, Port Talbot, SA12 7BR for a period of 3 weeks from 27th November 2025, to anyone who wishes may take a copy of this report or take extracts therefrom. Photocopies of the report or parts thereof will be provided on request free of charge.
