What is happening?
PUBLIC SERVICES OMBUDSMAN (WALES) ACT 2019
Notice pursuant to s.24(3) of the above Act.
The Public Services Ombudsman for Wales has investigated a complaint and issued a report on the results of their investigation which has identified service failure by Cwm Taf Morgannwg University Health Board.
The investigation focused on delays in the patient receiving hip surgery, due to administrative errors. In addition, communication failings with the patient and his GP regarding his removal from the waiting list were highlighted. It was concluded that there were missed opportunities to reinstate the patient on the waiting list.
The Health Board apologises for the failings identified within the report and confirms that recommendations to address the learning are being progressed.
A copy of the report will be available on the Health Board’s website (https://ctmuhb.nhs.wales) and for inspection by the public without charge during normal office hours at Ynysmeurig House, Navigation Park, Abercynon, CF45 4SN for a period of 3 weeks from 4 December 2025. Mae Ombwdsmon Gwasanaethau Cyhoeddus Cymru wedi ymchwilio i gŵyn ac wedi cyhoeddi adroddiad ar ganlyniadau eu hymchwiliad sydd wedi nodi methiant gwasanaeth gan Fwrdd Iechyd Prifysgol Cwm Taf Morgannwg.
Canolbwyntiodd yr ymchwiliad ar oedi cyn i’r claf gael llawdriniaeth ar ei glun, oherwydd gwallau gweinyddol. Yn ogystal, tynnwyd sylw at fethiannau cyfathrebu rhwng y claf a’i feddyg teulu ynghylch ei dynnu oddi ar y rhestr aros. Daethpwyd i’r casgliad bod cyfleoedd wedi’u colli i ail-gynnwys y claf ar y rhestr aros.
Mae’r Bwrdd Iechyd yn ymddiheuro am y methiannau a nodwyd yn yr adroddiad ac yn cadarnhau bod argymhellion i fynd i’r afael â’r gwersi yn cael eu datblygu.
