What is happening?

PUBLIC SERVICES OMBUDSMAN (WALES)
ACT 2019
Notice pursuant to s.24(3) of the above Act.
The Public Services Ombudsman for Wales has
investigated a complaint and found service
failure by Betsi Cadwaladr University Health
Board and has sent a report on the results of their
investigation to the Health Board. The complaint
related to failures in care and the provision of
timely treatment of a patient suffering from
carotid artery stenosis.
A copy of the report will be available on
the website www.bcu.wales.nhs.uk and for
inspection by the public without charge during
normal office hours at the Executive Offices
at Betsi Cadwaladr University Health Board,
Ysbyty Gwynedd, Penrhosgarnedd, Bangor,
Gwynedd, LL57 2PW for a period of three weeks
from 2 November 2023 and anyone who wishes
may take a copy of this report or make extracts
therefrom. Photocopies of the report or parts
thereof will be provided free of charge.

ganlyniadau eu hymchwiliad i’r Bwrdd Iechyd.
Roedd y gŵyn yn ymwneud â methiannau mewn
gofal a darpariaeth triniaeth amserol i glaf a oedd
yn dioddef o grebachiad rhydweli carotid.
Bydd copi o’r adroddiad ar gael ar y wefan
www.bcu.wales.nhs.uk ac yn Swyddfeydd
Gweithredol Bwrdd Iechyd Prifysgol Betsi
Cadwaladr, Ysbyty Gwynedd, Penrhosgarnedd,
Bangor, Gwynedd, LL57 2PW am gyfnod o dair
wythnos o 2 Tachwedd 2023 i’w archwilio gan y
cyhoedd yn ddi-dâl yn ystod oriau swyddfa arferol;
a chaiff unrhyw un sy’n dymuno gwneud hynny
gopïo’r adroddiad hwn neu wneud dyfyniadau
ohono. Darperir llungopïau o’r adroddiad neu
rannau ohono yn rhad ac am ddim.
