What is happening?

PUBLIC SERVICES OMBUDSMAN (WALES) ACT 2019

Notice pursuant to s.24(3) of the above Act.

The Public Services Ombudsman for Wales has investigated a complaint and found maladministration/ service failure by Betsi Cadwaladr University Health Board and has sent a report on the results of their investigation to the Health Board. The complaint related to failings in appropriate consent for treatment and care provided by an NHS trust commissioned by the Health Board and the Health Board’s lack of effective contract monitoring processes to ensure patient safety and to monitor the quality of commissioned services being delivered.

A copy of the report will be available on the website www. bcu.wales.nhs.uk and for inspection by the public without charge during normal office hours at the Executive Offices at Betsi Cadwaladr University Health Board, Ysbyty Gwynedd, Penrhosgarnedd, Bangor, Gwynedd, LL57 2PW for a period of three weeks from 8 April 2025 and anyone who wishes may take a copy of this report or make extracts therefrom. Photocopies of the report or parts thereof will be provided free of charge.

Mae Ombwdsmon Gwasanaethau Cyhoeddus Cymru wedi ymchwilio i gŵyn ac wedi canfod camweinyddiaeth/ methiant yng ngwasanaeth Bwrdd Iechyd Prifysgol Betsi Cadwaladr, ac wedi anfon adroddiad ar ganlyniadau eu hymchwiliad i’r Bwrdd Iechyd. Roedd y gŵyn yn ymwneud â methiannau o ran cydsynio priodol ar gyfer triniaethau a gofal a ddarperir gan ymddiriedolaeth GIG a gomisiynwyd gan y Bwrdd Iechyd a diffyg prosesau monitro contractau effeithiol y Bwrdd Iechyd i sicrhau diogelwch cleifion ac i fonitro ansawdd y gwasanaethau a gomisiynir a ddarperir. Bydd copi o’r adroddiad ar gael ar wefan www.bipbc.gig. cymru i’w archwilio gan y cyhoedd yn ddi-dâl yn ystod oriau swyddfa arferol yn Swyddfa’r Gweithredwyr, Bwrdd Iechyd Prifysgol Betsi Cadwaladr, Ysbyty Gwynedd, Penrhosgarnedd, Bangor, Gwynedd LL57 2PW am gyfnod o dair wythnos o 8 Ebrill 2025 ymlaen. Caiff unrhyw un sy’n dymuno gwneud copi o’r adroddiad hwn neu ddetholiadau ohono, wneud hynny. Ni chodir tâl am lungopïau o’r adroddiad neu rannau ohono.
