Notice is given that: Faizan Karim has applied to the London Borough of Hammersmith & Fulham for the Grant of a PREMISES LICENCE for the premises at: 794 Fulham Road, London, SW65SL known as: Koocina for the following licensable activities:

It is an offence liable on conviction to a fine up to Level 5 on the standard scale under Section 158 of the Licensing Act 2003 to knowingly or recklessly make a false statement in connection with this application.
