I, Marta Monika KACZMAREK have applied to Amber Valley Borough Council on 17.12.24 for the grant of a Premises Licence at Jezyk, Unit 1, Institute Ln, Severn Sq, Alfreton, DE55 7BQ for the following licensable activities: The sale of alcohol (OFF the premises) & Opening hours 08:00 – 22:00 Sun-Thu & 08:00-23:00 Fri-Sat.

It is an offence knowingly or recklessly to make a false statement in connection with this application, the maximum fine on summary conviction being unlimited.
